Ordered online and waited half an hour for pickup, drove all the way to the restaurant to be told that the order hadn't made it to the kitchen, someone had accepted the order and not entered it into their iPad system. There was a dietary requirement which couldn't be met for one dish so instead of calling me they didn't process it.
